Before playing the file:

• This is an actual dictation of an operative report played at actual speed by a doctor in a hospital system we're not at liberty to disclose. Certain information, in the interest of confidentiality, has been blanked; when blanks occur, simply leave a blank using the underscore or space bar.
• Open your RealAudio player now. Make sure your player is accessible, e.g., beside, beneath or on top of your browser. Your RealAudio player should have, all on the same row, the Play/Pause function, the Stop function, the forward/reverse slide bar, and the rewind/forward arrows. If it doesn't have these, get a newer RealAudio player (unless you feel you can do this without rewind). If you get behind, click the rewind button once or twice.
• As you can see, there is a typing area where we've included a kind of template so you won't have to type headings.
• Type your name and email address now (without an email address you won't receive your typed information). Once you've submitted your typing, your finished report will be sent to you immediately by email if you've given the correct email address, and the key will be returned to your browser.
• Center the controls and typing area now, and play the file according to your modem speed. The "Check Your Work" button is at the bottom of the typing area.

Important Note: If you want your results e-mailed to you, include your e-mail in the form below. Please be accurate when entering your e-mail address, as an incorrect e-mail address can generate a lot of unnecessary e-mail server activity. If you don't enter an e-mail address but still click on "Check Your Work" button, you will not be shown the correct spellings - you will just be told that the form is missing an e-mail address. A list of the correctly spelled words, next to your spelling of the words, is only available if you use a legitimate e-mail address. Thank you.
